New Patient Registration

register_nowPatients registering with the practice will be asked to complete various forms to enable us to ascertain details of your medical history. You will be invited to make an appointment for a new patient medical with the practice nurse. This is important as it enables us to record full details of your medical history, take up-to-date blood pressure readings and discuss your general health with you.

You will be asked for a urine sample for testing as well as information about any family history of illness and chronic conditions.

Children under five years of age do not need to have a medical but parents will be asked for details of all immunisations dates. The doctors at North Leverton Surgery encourage all children to have full immunisations.

All patients are registered with the practice rather than a particular doctor.
